Apple Valley Village Health Care Center, a Cassia Community, is hiring a Healthcare Appointment/Transportation Coordinator to join our team! As a Healthcare Appointment/Transportation Coordinator at Apple Valley Village Health Care Center, you will schedule appointments/ telehealth visits and corresponding transportation with community providers for facility residents on both TCU and LTC. Our ideal candidate has prior experience working in healthcare and/or patient scheduling. The ideal person for this role is organized, detail-oriented, and has excellent communication skills.
Position Type: Full-Time, benefits eligible position working Monday - Friday
Shift Available: 8:00 AM - 4:30 PM
Wage Range: $24.00 - $26.00 / hour depending on experience

Healthcare Appointment/ Transportation Coordinator Responsibilities:
- Schedule appointments/telehealth visits and corresponding transportation with community providers for facility residents on both TCU and LTC.
- Provide excellent customer service to residents, family members and community providers.
- Assist with uploading documents into EHR.
- Fax census activity report to pharmacy.
- Complete discharge census and discharge summary observations.
- Perform additional tasks as needed.
Healthcare Appointment/ Transportation Coordinator Qualifications:
- Prior experience in healthcare and/or patient scheduling preferred.
- Must possess excellent customer service and communication skills.
- Ability to work independently and meet deadlines.
- Must be very detail oriented and able to work with multiple interruptions.
- Working knowledge of pertinent State and Federal regulations.
