Job Description

JOB SUMMARY

The Intake Coordinator is responsible for managing all aspects of the patient intake process including communicating directly with patients and families, data entry, establishing and maintaining positive relationships with customers and referral sources.

DUTIES & RESPONSIBILITIES

  • 1.    
  • Assists with daily patient referral process.
  • 2.    
  • Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
  • 3.    
  • Monitor and process all incoming faxes 
  • 4.    
  • Implements improved work methods and procedures to ensure patients are admitted in accordance with policy.
  • 5.    
  • Reviews, organize referral documents, verify signature and input into HCHB system
  • 6.    
  • Verify and scan start of care packets with all required documents necessary for compliance
  • 7.    
  • N’tuc referrals that are not accepted
  • 8.    
  • Multiplatform scanning 
  • 9.    
  • Ensures input of new referrals as directed 
  • 10.  
  • Establishes and maintains positive working relationships with current and potential 
  • referral sources.
  • 11.    
  • Gathers, collates, and reports referral statistics including key customer referral trends.
  • 12.    
  • Maintains comprehensive working knowledge of Hospice and Home Health contractual relationships and ensures that patients are admitted according to contract provisions.
  • 13.    
  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by PASCO/SW Healthcare. 
  • 14.    
  • Attends and participates in staff, education, and agency committee meetings as appropriate
  • 15.    
  • Assists in all therapy and skilled nursing referrals and coordination 
  • 16.    
  • Participates actively in the Quality Improvement plan established by the agency 
  • 17.    
  • Performs other duties as assigned. 
  • 18.    
  • Performs audits on HCBS client charts
  • 19.    
  • Responsible for updating HCBS client spreadsheet
  • 20.    
  • Responsible for data entry of HCBS referrals and scanning appropriate documents into client charts
  • 21.    
  • Responsible for non-medical workflow in Home Care Home Base.

    JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

  • 1.    
  • The Intake Coordinator must have healthcare experience, preferably in referrals/intake in a home health or hospice environment.
  • 2.    
  • A nursing background is preferred but not required. 
  • 3.    
  • Demonstrates good communication, negotiation, and public relations skills.
  • 4.    
  • Demonstrates autonomy, assertiveness, flexibility, and cooperation in performing job responsibilities.
