Summary

Under the supervision of and in collaboration with the RN, performs responsibilities exercising initiative, judgment and independent decision-making within the scope of the State Nurse Practice Act, hospital policy and other governing organizations.

Job Duties

• Demonstrates adaptability and flexibility to changing situations including cares, medication and behaviors.*

• Identifies changes and trends in resident status. Individualizes care plans for residents depending on needs.Monitors resident outcomes and effectiveness of plan of care. Documents all findings.*

• Practices safety and infection control measures.Completes treatments, med administration and equipment use following established standards.Responds appropriately to emergency situations.*

• Observe for resident/family learning needs.Plans and coordinates with other disciplines (P.T., Pharmacist, etc.) to meet learning needs of residents.Teaches, counsels and reinforces residents and family members.*

• Assumes accountability and responsibility for quality of resident care provided. Demonstrates initiative and good judgment in all situations.*

• Fosters support and team building with residents & families, RCA's, and manager.*

• Ordering nursing supplies

Required Education and Experience

• Minnesota Licensureas a Licensed Practical Nurse

• Basic Life Support/CPR certification

• Experience in Assisted Nursing Facility
