Pay Range: $30.00-$34.00 DOQ

Gracewood Senior Living is seeking an LPN for our Lino Lakes location.The Licensed Practical Nurse is responsible for performing a variety of nursing duties including the implementation of resident's individualized care plans, monitoring resident health status, documentation and reporting of changes in resident's condition.  The LPN will also ensure that the home care services are in compliance with current regulations and standards.Choosing to work with us, means you:

Licensed Assisted Living Director responsibilities may include

  • Communicate with prospective residents, families, or current healthcare providers, if possible, to gather information about the resident's needs.
  • Assist the RN in ensuring residents have up-to-date service plans.  This includes reassessing the resident and modifying the service plan if necessary whenever the resident has a fall or other incident, or there is a significant change in the resident condition, or when the resident or a responsible party requests a change in services.
  • Monitor and maintain residents' clinical charts and clinical progress notes.
  • Ensure that staff clearly understands and follows all resident care procedures.
  • Communicate resident needs or condition changes to physicians and other personnel (e.g., caseworkers and health plan care coordinators).
  • Facilitate coordination of resident care services with other service providers.
  • Keep assessments up to date per policy.
  • Order medications, all medication renewals, and any necessary follow-up with physicians regarding medications and medical conditions.  This may include weekly medication setup for unlicensed staff, as necessary.
  • Ensure all physician orders are signed and implemented within 24 hours or the next nursing duty day of receipt.
  • Ensure that medication administration procedures are followed properly, investigates any errors and issues with staff, and implement corrective action plans as needed.
  • Ensure new medication sheets are in place by the first of each month and/or monitor the Eldermark Pending Review to ensure residents receive medications appropriately.
  • Review medication administration documentation to ensure accurate and appropriate documentation.
  • Document disposal of medication per procedure each week or per policy
  • Supervise medication practices and medical equipment being used in the building.
  • Communicate in the communication book any information that staff needs to be aware of.
  • Monitor staff for accuracy of resident vital signs and follow-up appropriately.
  • Work with the Assisted Living Director to coordinate and conduct monthly in-service/staff meetings.
  • Work with Assisted Living Director to ensure emergency procedures are in place and staff is trained on these procedures.
  • Ensure compliance with regulations and is available for state licensure surveys.
  • Investigate and report incidents of suspected resident neglect and abuse and report these findings to appropriate county officials in coordination with the Assisted Living Director.
  • Share resident and family concerns with pertinent staff if appropriate.
  • Maintain a positive relationship with the residents, their families, housing and home care staff, and the larger community.  This includes representing the building at community events and events held in surrounding communities.
  • Attend monthly in-service/staff meetings as requested and required.
  • Available for on-call medical questions and emergencies via phone, as needed.
  • Assist in investigating complaints about facility services submitted by residents or others, identifying steps to address the complaint, and communicating with the residents about the company's actions to resolve the issue if requested.
  • Perform other duties and assist with other company projects, as requested.

Qualifications:

  • Graduate of an accredited school of nursing 
  • Must be registered and licensed with the State of working as an LPN and must maintain a current unencumbered license with the Board of Nursing
  • Experience within a long-term care facility or home healthcare preferred
  • Excellent judgment and decision-making abilities and problem-solving skills
  • Flexibility in dealing with changing situations
  • The ability to deal tactfully and use good communication and customer service skills with staff, residents, and their families both verbally and in writing
  • Basic computer skills and ability to use spreadsheet applications and electronic records
  • Ability to lift up to 50 pounds
  • Must pass criminal background from the Department of Human Services at the time of hire and every four years after
  • Must be screened for and pass a TB test
