Scheduled Hours: 8:00am - 4:30pm, every 8th weekend rotation required.

Job Description:

A Brief Overview

Provide patients and their families, including vulnerable populations, with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services address their social, emotional, and financial needs. Services provided include clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community. These services advise family caregivers, provide patient education and counseling, and make necessary referrals to community resources for continuity of care.

What you will do

  • Advocate for clients or patients to resolve crises.
  • Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs.
  • Counsel clients, patients and their families to help them adjust to diagnosis, prognosis, and hopsice admission.
  • Assist director and volunteer coordinator with community programs to promote public relations and community awareness.
  • Identify environmental impediments to client or patient progress through interviews and review of patient records, and provide possilbe solutions.
  • Modify treatment plans to comply with changes in clients' status.
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
  • Assist family members in understanding, dealing with, and supporting the client or patient.
  • Collaborates with the discharge planning team to facilitate and arrange appropriate discharge needs of the client according to standards set by Joint Commission and CMS.
  • Regular and reliable attendance.

Qualifications

  • Bachelor's Degree in Arts/Sciences (BA/BS) Bachelor's degree in Social Work from a program accredited by the Council of Social Work Education or LBSW Preferred
  • Bachelor's Degree in Arts/Sciences (BA/BS) Bachelor's degree in a Human Services-related field, ie. Psychology, Sociology, Criminal Justice, etc., Required
  • Social Work experience in health care settings Preferred
  • Social Work experience in other practice settings Preferred
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for counseling and guidance.
  • Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures and their history and origins.
  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
  • Being aware of others' reactions and understanding why they react as they do.
  • Talking to others to convey information effectively.
  • Actively looking for ways to help people.
  • Understanding written sentences and paragraphs in work related documents.
  • Ability to speak to others in a professional manner.
  • The ability to listen to and understand information and ideas presented through spoken words and sentences.
  • The ability to communicate information and ideas in speaking so others will understand.
  • The ability to apply general rules to specific problems to produce answers that make sense.
  • The ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem, only recognizing there is a problem.
  • The ability to read and understand information and ideas presented in writing.

Palliative Care, West Burlington:

  • Risk exposure to airborne pathogens is a level 1 category, meaning the job requires you to enter airborne isolation rooms (N95 mask fit).
