At a Glance Flexible scheduling — you decide when and where the work happens, as long as deadlines and visit requirements are met. Travel is part of the job — this is a five-county footprint across Maryland's Eastern Shore. A reliable vehicle and valid driver's license are required. Strong organization is essential — you'll be tracking multiple clients, deadlines, consents, and authorizations across several sites without daily oversight. Technology comfort is essential — you'll be working in an electronic health record, conducting some sessions by secure video, and using electronic consent and authorization workflows. Independent, not isolated — you'll have a supervisor and a team of case managers to coordinate with, but the day-to-day pace is yours to manage. |
Position Summary
The Targeted Case Management Community Support Specialist conducts comprehensive TCM assessments and develops individualized care plans for new referrals and required six-month reassessments. This role focuses on assessment, treatment planning, and the documentation, consents, and authorizations that keep our clients' services active and compliant.
You will partner with our case managers, supervisors, and referral sources to identify each client's needs and translate those needs into a clear, achievable plan. You will own the timelines for your own assessments and reassessments, the authorizations tied to them, and the consents and releases that make coordination of care possible.
This is a contractual position with flexible scheduling. You decide when the work happens — within the deadlines set by Maryland Medicaid, our Administrative Service Organization (ASO), and program requirements. Travel across our Eastern Shore clinics and into clients' homes and communities is a regular part of the role.

This position requires regular travel across CBH's Eastern Shore footprint. Sites and counties served include:
Assessments and reassessments take place in CBH clinics, in clients' homes, and in community settings. A reliable personal vehicle, valid Maryland driver's license, and current auto insurance are required. Mileage reimbursement is provided in accordance with agency policy.
Schedule
This is a contractual role with flexible scheduling. You set your own hours within the deadlines defined by Maryland Medicaid, the ASO, our supervisor, and the program. Evening or weekend work is at your discretion. There is no required clock-in time and no minimum daily presence in any single clinic location — but every assessment, care plan, consent, and authorization must be completed on time.
Compensation
Flat rate of $48 per completed note. This is a fee-for-service contractual position; the rate is all-inclusive and covers travel, the visit, the authorization, and the documentation.
