Job Overview

We are seeking a dedicated Travel Licensed Practical Nurse (LPN) for a short-term assignment in a skilled nursing and rehabilitation facility. The ideal candidate will provide direct patient care, administer medications, monitor resident health status, and support the interdisciplinary care team in delivering high-quality outcomes.

This assignment is well-suited for LPNs looking for short travel contracts in a structured long-term care environment.

Key Responsibilities

• Provide direct nursing care to residents according to individualized care plans• Administer medications and treatments as prescribed• Monitor, assess, and document patient conditions and changes• Assist with admissions, discharges, and resident assessments• Collaborate with RNs, CNAs, physicians, and interdisciplinary team members• Maintain accurate and timely clinical documentation• Ensure compliance with infection control and safety protocols• Deliver compassionate, patient-centered care

Guaranteed Hours: 40 Hours per Week

Requirements

• Active Licensed Practical Nurse (LPN) license required• Previous long-term care or skilled nursing experience preferred• Strong clinical assessment and communication skills• Ability to work independently and within a healthcare team• Must have reliable transportation• Must comply with facility onboarding requirements• Must bring state or federally issued identification and wear a photo badge at all times• COVID-19 vaccination or approved exemption may be required per facility policy
