Description

UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are currently seeking a full-time (40hr) Early Childhood Care Coordinator who will be responsible for providing support services to families with high-risk children, regarding behavioral or emotional problems, parenting issues, and life stressors by assisting in the development of a comprehensive, integrated, culturally responsive family-driven plan that includes the young child and all family members.

This position is eligible for a sign-on bonus of $7,000. Position will be hybrid.

Essential Responsibilities -

  • Provide outreach and engagement to client upon referral to the program
  • Provide weekly face to face direct service hours to client including support, advocacy and assistance with the development of positive parenting, safety planning, basic life skills and parenting a child with special needs
  • Arrange Child and Family Team Meetings with family determined supports and providers and follow through recommendations made at these meetings
  • Work collaboratively with the Early Childhood/Developmental Clinician in a team approach
  • Participate in case reviews and supervision; attend all regularly scheduled team meetings and training; maintain familiarity with community resources and collaborate with other agencies such as DCF and other community based supports that are involved with the family
  • Maintain up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations and termination summaries

Minimum of 60% travel may be required.

Requirements

  • Bachelor's Degree with one to three years prior experience or training in social services
  • Experience working with young children and families with significant behavioral health needs.
  • Ability to provide a strength based, client centered approach to the delivery of service
