JOB SUMMARY:

The Certified Nursing Assistant (CNA) will provide personalized healthcare to members under the direction of the clinical program manager or clinical team lead. The CNA supports the NP or PA's plan of care delivery and monitoring, alerting the NP/PA of any concerns or status updates. This position serves adults patients with acute and chronic medical conditions. Addressing the psychosocial needs of the members is a key component of the position.

ESSENTIAL JOB DUTIES:

To perform this job, an individual must accomplish each essential function satisfactorily, with or without a reasonable accommodation.

  • Recognize and respond to the needs of members
  • Provide medical care within the scope of training of licensure and in consultation with the covering NP/PA
  • Observe residents' conditions, vital signs, activities of daily living, and report changes to the covering NP/PA as soon as possible
  • Participate and assist with recreational and therapeutic activities for residents
  • Work with RN Case Managers as part of an integrated care team (as applicable)
  • Participate in the development of an individualized care plan and attend interdisciplinary Team meetings as needed
  • Attend weekly case conferences as needed
  • Conduct informal facility staff education and teaching to improve the effectiveness and efficiency of care delivery
  • Other duties as assigned by supervisor

JOB REQUIREMENTS:

  • Maintain thorough and timely documentation of services rendered in the member's chart
  • Practice in a cost-effective manner
  • Adhere to policies and procedures relating to the provision of clinical and related services
  • Ability to effectively communicate and interact with multicultural and multi-age populations with a strong focus on customer service
  • Ability to adapt to change and manage time efficiently
  • Self-directed and able to work with minimal supervision
  • Adhere to and respect all policies regarding member's rights, anonymity, and confidentiality of all resident records past and present
  • Follow infectious disease precautions to prevent spread of infections
  • Must be willing and able to work flexible hours
  • Travel required

  • Current automobile insurance according to company policy

REQUIRED QUALIFICATIONS:

  • License/Certification/Education:

o High school diploma or equivalent required

o Current state Certified Nursing Assistant license

o State driver's license

o CPR certification preferred

  • Experience:

o Six (6) months' healthcare experience preferred

SCOPE AND LEVEL OF WORK:

  • Complexity: Duties are not generally complex. Work assignment is performed within an established framework under general instructions but requires simultaneous coordination of assigned functions or projects in various stages of completion.
  • Decision Making: Exercises judgment and discretion; is not a decision maker.
  • Communications: Contacts with the public or employees where explanatory or interpretive information is exchanged, defended, and gathered and discretion and judgment are required within the parameters of the job function.
  • Supervision Received: Performs work with supervisory oversight. Under general supervision, receives assignments and is expected to carry them through to completion with substantial independence. Work is reviewed for adherence to instructions, accuracy, completeness, and conformance to standard practice or precedent. Recurring work clearly covered by guidelines may or may not be reviewed.

  • Position may require flexible hours, unscheduled overtime or occasional week-end work
  • Must be able to speak and write in English
  • Travel required
