Responsibilities:
- On Site services and support
- Communicating observations and information about the consumer resident to the Program Supervisor, RN and treatment team
- Supervising, organizing and supporting recreational and/or socialization activities
- Transportation to any scheduled appointment, partial care and extracurricular activities;
- Crisis Intervention services
- Participation in individual service coordination
- Documenting observations, information and services provided to the consumer resident in
- Assist with the development of initial and comprehensive treatment plans; assist with the completion of initial
- intake assessment
- Respond to emergency situations as directed by Program Supervisor
- Have flexibility to be pulled to work in other residential group homes
- Respond to emergency situations as directed by the Program Supervisor
- Availability to consumers and staff on a 24 hour emergency basis

Qualifications:
- Bachelor's (BA/BS) Degree from an accredited college in a mental health related discipline plus one year of experience in a residential Mental Health setting; OR
- Bachelor's (BA/BS) degree in an unrelated field plus one year related work or life experience and one year of experience in a residential Mental Health setting; OR
- a combination of one or more years of college plus work or life experience equal to four years plus one year of experience in a residential Mental Health setting; OR
- High School Diploma or equivalent plus four years of related work or life experience plus one year of experience in a residential Mental Health setting;
- Good verbal and writing skills;
