We are seeking a Medical Social Worker for either part-time (minimum 20 hours/week) or contingent/as-needed (flexible schedule) roles. In this position, you will deliver professional social services to patients based on physician orders, adhering to state and federal regulations as well as agency policies. This is a vital role that ensures compassionate, patient-centered care for those in need.
Key Responsibilities
- Provide high-quality social services to patients in their homes.
- Collaborate with the healthcare team to ensure seamless continuity of care.
- Support patients and families in navigating the challenges of illness and treatment.
Qualifications
- Master's Degree in Social Work from an accredited institution.
- Minimum of one year of experience in a healthcare setting.
- Current Social Worker license in the state of Illinois (or applicable registration).
- Strong commitment to professional development and ethical standards.
- Valid driver's license and reliable transportation.
- Preferred: Experience in home care or hospice settings.
