About the Role:

Job Summary

Working under the supervision of the Nurse Manager Sepsis Program, the RN Navigator: Sepsis Program provides concurrent review of Emergency Department (ED) and admitted patients to identify at risk patients for sepsis and provides support to the patient care team to ensure timely identification of sepsis, timely initiation of appropriate sepsis care, and identification of barriers to timely care. The RN Navigator rounds on high risk sepsis patients for timely identification of deterioration for intervention and provides guidance on laboratory results to ensure appropriate de-escalation of care. The RN Navigator will provide patient and family education and provides sepsis discharge plans for comprehensive coordination of care. Assists in the work of the Regional Sepsis Collaborative. Assists in the advancement of the professional practice environment by communicating the NGMC nursing strategic direction and focusing on activities that support the nursing strategic direction. Provides support for VOICE and the NDNQI quality teams as necessary within the department. Assists with overall professional nursing practice and other projects as assigned.

Minimum Job Qualifications

  • Licensure or other certifications: Licensed to practice as a RN in Georgia. Professional certification in either a clinical or other relevant specialty within two (2) years of hire.

  • Educational Requirements: A degree in Nursing (either BSN or MSN).

  • Minimum Experience: Two (2) years of healthcare experience in a related nursing field (i.e., bedside primary nursing, case management nursing, care manager nursing, quality improvement nursing).

  • Other: Professional organization membership within three months of hire into the role.

  • Preferred Educational Requirements: A Masters Degree (Nursing or other if possessing a BSN) within five (5) years.

Essential Tasks and Responsibilities

  • Primarily responsible for concurrent review of ED and admitted patients to ensure timely identification of sepsis, timely initiation of treatment, and utilization of evidence-based treatment.

  • Responsible for ensuring effective communication to clinical modalities, patients, families, communities, interdisciplinary team members, and physicians regarding a patient's sepsis risk and appropriate actions associated with identified risk.

  • Demonstrates excellent detail management as well as possesses strong self-motivational skills; demonstrating consistent, reliable, credible, respectful and timely service to patients, families, physicians, and nurses.

  • Understands and is able to utilize all computer systems accurately and efficiently.

  • Ability to communicate and implement (in an accurate and timely manner) process change as needed to align with changes in sepsis best practice.

  • Responsible for tracking/trending errors and compliance with sepsis bundles and organizational sepsis standards. Willingly participates in education and training efforts to maintain knowledge level in a constantly changing environment.

  • Adjusts routines and schedules necessary in order to complete essential responsibilities across all NGMC entities.

  • Reviews daily quality, productivity, and other reporting and provides communication regarding any concern or trends to management and employees.

  • Assists in managing resources within budgetary constraints (supply ordering, patient education content, patient materials, inpatient education, and discharge materials).

  • Utilizes time management skills to ensure completion of daily responsibilities.

  • Assists case management and the attending patient care team in identifying appropriate resources. This includes providing the patient with a sepsis discharge plan necessary for optimal transition of care.

  • Collect and report patient information from patient and EHR to assess sepsis care on patient outcomes.

  • Manages the Sepsis Risk Worklist to identify high risk patients for inclusion in patient rounding.

  • Serves as a delegate for the Nurse Manager Sepsis Program in meetings related to sepsis and assists with agendas, taking meeting minutes, and handouts for various program meetings (Sepsis Core Team, Ambulatory Sepsis Workgroup, etc).

  • In addition to the above criteria, the RN Navigator: Sepsis Program will be responsible for and must be proficient in all job duties. Those duties include, but are not limited to knowledge of sepsis risk paradigms, sepsis best practice treatment bundles, and CMS Sepsis Core Measure guidelines and regulatory requirements.
