Job Description

Base salary starts at $30.65 / hr.
E S S E N T I A L F U N C T I O N S
- Coordinate with provider and clinical team to identify, educate, and assist patients in enrolling for CCM and/or BHI program.
- Manage a roster of patients enrolled in the CCM/BHI program to include monitoring their after visit care and supporting their short/long term health goals.
- Care planning for all health issues including systems to ensure receipt of all recommended preventive services. Assist patient with scheduling diagnostic testing and referrals.
- For behavioral health integration (BHI); assessment and care management services including the administration of validated rating scales, behavioral health care planning in relation to behavioral/psychiatric health problems, including revision for patients who are not progressing or whose status changes; provision of brief psychosocial interventions; and ongoing collaboration with primary care practitioner, all in consultation with psychiatric consultant.
- Ability to utilize EMR to record and track patient interactions for CCM/BHI.
- Ability to lead an interdisciplinary care team and work independently as needed.
- Strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner.
- Other duties as assigned.
E D U C A T I O N
- Bachelor’s degree in Social Work required.
E X P E R I E N C E
- One (1) year work or equivalent educational experience as a social worker in a healthcare setting preferred; experience coordinating care for patients and/or experience in behavior health integration also preferred.
C R E D E N T I A L S
- Must have a valid, current license to practice Social Work in the State of Wyoming and must be in good standing with the Mental Health Professions Licensing Board.
- Must have a current, valid Licensed Clinical Social Worker in the State of Wyoming and must be in good standing with the Mental Health Professions Licensing Board.
- Must have a valid, current Basic Life Support (BLS) certification or must obtain with 60 days of hire and maintain credentialing throughout employment at IMH.
