Job Description
Job Title: Licensed Clinical Social Worker (LCSW), Inpatient & Emergency Department Services
Position Summary:
We are seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) with a Master of Social Work (MSW) degree to join our multidisciplinary team. This role provides non-clinical, biopsychosocial assessments and interventions across inpatient units and the outpatient Emergency Department. The LCSW will support patients and families through crisis intervention, high-risk screening, focused emotional support, discharge planning, and coordination of essential resources.
The ideal candidate will demonstrate a strong commitment to patient- and family-centered care, exhibit creative and compassionate problem-solving skills, and uphold the core values of dignity, respect, and autonomy in all interactions.
Key Responsibilities:
Collaborate closely with RN case managers, acute care nurses, physicians, and other healthcare professionals to assess and address patients’ psychosocial and discharge planning needs.
Identify and mitigate barriers to recovery and safe discharge, including emotional, financial, psychosocial, medical, and spiritual concerns.
Maintain a comprehensive understanding of Medicare, Medicaid, and private insurance policies, including managed care principles and utilization management.
Perform thorough psychosocial assessments and develop individualized discharge plans for high-risk patients, especially those with behavioral health or complex social needs.
Facilitate family meetings and interdisciplinary care conferences to support patient-centered decision-making.
Provide counseling, emotional support, and resource referrals to patients and families navigating chronic, acute, or terminal illness.
Deliver crisis intervention services and provide supportive counseling in emotionally charged situations, including end-of-life care and death notifications.
Respond to non-discharge social work consults throughout the facility, including specialized units such as ICU, Trauma, Oncology, NICU, Transplant, and the Emergency Department.
Qualifications:
Master of Social Work (MSW) from an accredited program.
Active Licensed Clinical Social Worker (LCSW) license required.
Minimum of 2 years of social work experience (required for LCSW eligibility).
Prior experience in a hospital or healthcare setting is strongly preferred; however, candidates with strong community-based social work experience will also be considered.
School social work experience will not be considered for this role
