Position Summary
We are seeking a Licensed Master Social Worker (LMSW) to join a Review Company based in New Rochelle, NY as a Nursing Home Surveyor/Complaint Investigator. This full-time, contract role focuses on conducting nursing home surveys, investigating complaints, and ensuring compliance with federal and state regulations.
Responsibilities
- Conduct nursing home surveys and complaint investigations, including those involving abuse or neglect
- Evaluate facilities’ compliance with state and federal regulations
- Draft Statements of Deficiencies (SOD) using the Principles of Documentation
- Review and assess corrective action plans for compliance
- Complete required data entry in state and federal reporting databases
- Participate in immediate jeopardy monitoring and state oversight activities
- Provide testimony in administrative hearings as needed
- Attend required meetings and adhere to DOH and IPRO schedules and policies

Requirements
- Licensure: Active New York State LMSW license
- Experience:
- Minimum of 2 years of clinical experience in a medical facility, preferably a nursing home (required)
- 2 years of experience conducting nursing home surveillance activities (preferred)
- Certification:
- SMQT Certification (preferred; must be willing to obtain within 12 months)
- Other:
- Must be able to travel to on-site nursing home locations as required
- Strong documentation and regulatory compliance skills
