Title
VP Post Acute Services
The Vice President, Post-Acute Services provides strategic, operational, and clinical leadership across Bronson Healthcare's post-acute continuum, including our Skilled Nursing Facility, Bronson at Home, Case Management/Care Management and overall Managed Care partnerships. This role is responsible for advancing systemwide performance in care transitions, quality, regulatory compliance, and value-based care integration. The leader will develop and execute strategies to enhance care coordination, patient outcomes, payer relationships, and the financial sustainability of post-acute operations.
As a key member of the senior leadership team, the Vice President ensures that Bronson's post-acute services align with system goals and deliver exceptional care experiences while preparing for emerging reimbursement models.
Key Responsibilities
Strategic Leadership
- Develop and implement a multi-year strategic plan for post-acute services in alignment with Bronson's enterprise strategy and community health goals.
- Lead efforts to integrate post-acute services into population health initiatives, ACO models, and payer-provider partnerships.
- Represent post-acute services in system-wide strategic planning, budgeting, and clinical integration initiatives.
Operational & Clinical Oversight
- Provide executive oversight for all post-acute service lines including skilled nursing, home health, case management and care management services.
- Ensure compliance with all state and federal regulations (e.g., CMS Conditions of Participation, Joint Commission, MDS, PDPM).
- Oversee development of systems and processes that improve access, quality, safety, and efficiency across the continuum.
- Utilize data and analytics to drive continuous improvement in clinical outcomes, patient satisfaction, employee engagement, and cost of care.
Payer Strategy & Financial Performance
- Expand and strengthen Bronson's post-acute network through managed care partnerships and innovative contracting.
- Collaborate with Finance and Managed Care to optimize reimbursement under PDGM, PDPM, and Medicare Advantage models.
- Maintain oversight of service line budgets, capital planning, and long-term financial sustainability.
Talent & Culture
- Lead, mentor, and develop a high-performing team of directors and managers across post-acute services.
- Foster a culture of accountability, collaboration, compassion, and excellence.
- Champion diversity, equity, and inclusion within leadership pipelines and workforce strategy.
Governance, Stakeholder Engagement & Partnerships
- Serve as liaison to the Board of Directors on matters related to post-acute strategy and performance.
- Cultivate relationships with physicians, payers, referral sources, and community organizations to strengthen care coordination.
- Partner closely with hospital, ambulatory, and population health leaders to optimize transitions of care and system integration.
Required Qualifications
- Master's degree in Healthcare Administration, Nursing, Business, Gerontology, or related field.
- 10 years progressive leadership experience and exposure to post-acute operations, with direct oversight of skilled nursing and/or home health services is preferred.
- Demonstrated experience managing multi-site service lines, budgets, and clinical quality programs in a complex healthcare system.
- Licensed Nursing Home Administrator (NHA) in the state of Michigan or eligibility to obtain licensure within 6 months of hire is preferred.
Preferred Qualifications
- Certification as a Fellow of the American College of Health Care Administrators (FACHCA)
- Clinical licensure (RN, PT, OT, etc.)
- Experience in payer contracting, population health models, and value-based care delivery
