Job Description
Position Overview:
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our patient-centered care team. The PT is responsible for delivering high-quality, evidence-based therapeutic services to a designated group of patients in collaboration with physicians and interdisciplinary care teams. This role involves performing comprehensive assessments, developing and implementing individualized care plans, and guiding patients through the continuum of care with a focus on functional improvement, engagement, and an exceptional patient experience.
Key Responsibilities:
Conduct thorough evaluations and provide treatment interventions in accordance with state regulations and professional standards of practice.
Assess rehabilitative and functional status to help develop individualized, goal-oriented plans of care in collaboration with physicians and team members.
Deliver skilled physical therapy services in the patient’s home environment, ensuring treatment aligns with the physician’s plan of care and supports functional outcomes.
Educate patients, families, and caregivers on therapeutic techniques, home exercises, assistive devices, and/or prosthetics to support continued progress outside of therapy sessions.
Effectively manage caseload priorities, participate in daily staffing coordination, and facilitate smooth handoffs and communication among care team members.
Qualifications:
Bachelor’s Degree in Physical Therapy required (Doctorate preferred).
State Licensure as a Physical Therapist in [Insert State].
Basic Life Support (BLS) certification, or must be obtained within 30 days of employment.
