Job Description
Position Summary:
Under the supervision of the Community Based Nurse Manager works collaboratively with all members of the interdisciplinary healthcare team, provides consultation and supervision to the clinical site team (Water Street) and provides individual and group support and services to GLFHC patients with stimulant use disorder, helping them navigate the social systems and access necessary resources.
Scope of Duties:
- Conduct assessments to determine clients’ needs and develop individualized care plans for clients.
- Monitor and evaluate clients’ progress, adjusting service plans as necessary to ensure effective support.
- Provide short term interventions and support to clients and their families, addressing various social, emotional and behavioral issues.
- Collaborates with community behavioral health providers in the coordination of behavioral health care.
- Develops and facilitates support groups as determined by patient/client need, interest and team member recommendation.
- Collaborate with other service providers to coordinate care and ensure clients receive necessary services such as housing, healthcare, exercise and employment services.
- Advocate for clients and work to address systemic barriers to care.
- Maintain accurate and confidential client records and progress notes.
- Participate in continuing education and professional development opportunities to stay current with best practices in the field.
- Participate in team meetings and case conferences.
- Documents all contacts on a daily basis.
- Collaborates with GLFHC clinical staff and administration to develop and maintain referral resources throughout service area. Refers patients to services within GLFHC and to other community agencies.
- Develops working relationship with leaders and community agencies, representing GLFHC at meetings as indicated.
- Provides care with sensitivity to the patient’s level of understanding, culture, age and language.
- Demonstrates clinical competence and professional commitment by adhering to established standards of practice and utilizing appropriate scientific principles.
- Is a proponent of the improvement process and works cooperatively with team members in making necessary changes.
- Develops and maintains systems for patient educational materials and staff. On a regular basis participates in ongoing quality improvement activities related to individual, team, and organizational performance improvement.
- Collaborates with designated behavioral health professionals for patient care and regular case review.
- Under the direction of the Community Based Manager, designs, implements and evaluates a program for clients with Stimulant use disorder, following the clinical guidelines.
- Supervises the daily activities and staffing of both the stimulant and opioid drop in centers, including supervising staff schedules, staff activities, reporting, client flow and satisfaction.
- Assists with work plan development for both programs (ITLS and StART)
- Attends program meetings with CSS leadership, CSS team, Water street team, DPH, and BSAS
- Travel between sites may be required.
Requirements:
- Bilingual, Spanish/English required.
- BLS Certification required.
Education
- Requires Bachelor’s Degree in Social Work and/or Psychology from an accredited program.
