Social Worker
SHIFT: Per-Diem
JOB DESCRIPTION:
The Social Worker workers under the direction of the Administrator to ensure that the resident's medically related, emotional, and social needs are met and maintained individually. The Social Worker will have administrative authority, responsibility, and accountability for carrying out assigned duties.
RESPONSIBILITIES:
- Provides support to the residents and families in coordinating residents' needs to ensure needs and expectations are met during their stay
- Work closely with the interdisciplinary team to coordinate care and discharge plans
REQUIREMENTS:
- Minimum 18 years of age
- A Bachelor's Degree in Social Work or Human Services with experience in geriatrics
- Must have one year of supervised social work experience (paid/volunteer) in a healthcare setting, working directly with individuals to implement, evaluate, and direct the overall operation of the Social Service Department in accordance with current federal, state, and local standards, guidelines, and our established policies and procedures
- Strong written and verbal communication skills
- Respect for residents' privacy and rights; display of genuine concern and empathy for the elderly
- Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
