Job Description

WHAT YOU WILL DO:

The Outreach Liaison serves as a vital link between the Hospice House, hospitals, hospice physicians, hospice agencies, and the community. This role is responsible for identifying, assessing, and facilitating the admission of eligible patients into the Hospice House, particularly for general inpatient services (GIP) when appropriate and respite care. The IPU Outreach Clinical Liaison provides education to patients, families, and healthcare professionals about hospice philosophy, services, and benefits, ensuring a seamless transition of care and promoting the organization's mission of compassionate end-of-life care and family support.

  • As our Clinical Liaison, you will serve as the face of the Hospice House in the community and with healthcare providers.
  • You will:
  • Drive strategic growth by identifying referral opportunities and building lasting relationships with hospitals, physicians, case managers, and hospice agencies.
  • Serve as a trusted resource for patients and families-clearly and compassionately explaining hospice services and eligibility.
  • Coordinate care transitions through timely clinical assessments, collaboration with interdisciplinary teams, and execution of admissions.
  • Promote our mission at community events, professional conferences, and through educational presentations to referral sources.
  • Ensure excellence in care by supporting documentation, compliance, and a seamless patient experience from referral to admission.

WHAT YOU'LL BRING:

  • Licensed RN (BSN preferred) or experienced LPN in good standing (Virginia license required).
  • 2-3 years of clinical experience in hospice, palliative care, acute care, case management, or related fields.
  • Strong clinical assessment and communication skills.
  • Proven ability to build relationships and educate others about hospice care.
  • Proficiency with EMRs and CRM systems.
  • Compassion, resilience, and a patient-centered mindse
