We are hiring a Social Services Professional to help support clients' well-being and independence by providing social work consultation; coordinating client care, as well as develops and facilitates therapeutic group activities.
Our Social Services Professional:
Provides information to referral sources and potential clients by receiving intake inquiries and giving tours.
Identifies clients' service needs by gathering and completing intake information and working with the client, caregiver and/or referral source to develop service plans.
Provides team with social work expertise.
Helps maintain continuity of care among team members by providing communication, documentation, and support.
Encourages enrollment by maintaining contact with caregivers, referral sources and potential clients as well as site liaisons and site managers from other sites.
Maintains client confidence and protects operations by keeping information confidential.
Develops a safe, therapeutic group environment by establishing and keeping group rules; monitoring and encouraging appropriate participation; intervening as necessary.
Maintains required documentation to meet WAC guidelines and/or Full Life Care policy;
Performs other duties as assigned.
What you need to succeed!
- Master's degree in social work, gerontology, or other human services field, or counseling and at least one year of professional work experience (full-time equivalent), or a bachelor's degree in social work, counseling, or a related field and two years of experience in a human services field, or related field.
- Must complete the HIV/AIDS information course;
- Employees are required to receive an annual influenza vaccine unless granted a medical or religious exemption.
Physical Requirements for the role:
- Must be able to lift, push, and pull up to 25 lbs, may occasionally need to support client mobility.
