Job Description
Position Summary: This is a 12-month first-year postgraduate training program. The resident will be a self-directed, independent, motivated learner guided by the Residency Program Director and the appropriate preceptors for each learning experience. The resident will rotate through 12 months of inpatient and ambulatory patient care experiences to foster independent clinical decision making through established collaborative drug therapy management agreements. Primary clinical responsibilities of the resident include rounding with assigned teams, performing pharmacokinetic monitoring, validating pharmacy orders, performing medication reconciliation, ensuring appropriate transitions of care, and providing patient education. Ultimately, the resident is responsible for providing comprehensive medication therapy management for patients by designing, recommending, and monitoring therapeutic regimens that incorporate evidence-based medicine.
Essential Functions, Competencies, and Professional Requirements:
- Participates in daily medical and pharmacy teaching rounds and provides patient care pharmacotherapy services
- Collaborates with physicians, medical residents, and other prescribers to ensure safe and cost-effective medication use
- Provides drug information to healthcare professionals
- Monitors for medication efficacy and toxicity
- Provides therapeutic drug level monitoring
- Administers medication reconciliation and drug discharge counseling
- Provides pharmacokinetic, pharmacotherapeutic, infectious diseases, and pharmacy-related consultations
- Detects, reports, and manages adverse drug events and medication errors
- Documents pharmacotherapy interventions
- Conducts a medication-use evaluation and research project
- Presents regularly to clinical pharmacy staff at Pharmacotherapy Grand Rounds
- Completes regular evaluations in PharmAcademic
- Earns faculty appointment at Touro College of Pharmacy
- Serves as covering clinical pharmacist and/or staff pharmacist on roughly one evening per week and every third weekend
- Works professionally and respectfully and builds relationships with each multidisciplinary team
- Performs all other duties as required.
Annual Salary: $55, 000
Days: Mon - Fri (Alternate weekends)
Shift: 8am - 4pm
