Job Summary

Job Description

  • Serves as a link between the Parish Health Units (PHUs) and community programs and resources for the Bureau of Regional and Clinical Operations (BRCO).
  • Helps provide oversight to the execution of the Community HealthWays program in parishes within region. Helps supervise regional Community Health Coordinator 1 staff.
  • Monitors regional staff completion of screenings, needs assessments, action plans, and follow-up appointments with clients in meeting their identified goals.
  • Audits client encounters in detail and provide corrective action plans within the region.
  • Leads the creation and maintenance of a directory of community resources (e.g. food banks, housing assistance programs, childcare resources, etc.) in parishes within the region.
  • Develops partnerships and attend key meetings in parishes within region.
  • Oversees community engagement events in parishes within region.
  • Prepares reports and documents as needed or requested by program supervisors and Regional Leadership Team.
  • Leads regional group meetings with Community Health Coordinator 1 positions.
  • Attends scheduled program and regional meetings.
  • Other tasks as assigned.

QUALIFICATIONS

REQUIRED:

  • Bachelor's degree, or Associate's degree plus 3 years of professional experience, or 6 years of professional work experience.
  • Minimum 2 years professional experience within the healthcare, social services or community organization fields.
  • Excellent analytical and critical thinking skills; effective organizational and time management skills.
  • Great attention to detail and follow up.
  • Ability to manage projects, assignments, and competing priorities.
  • Proficient in the use of Zoom, Teams, and Microsoft Office, including but not limited to Outlook, Word, and Excel.

DESIRED:

  • Advanced degree.
  • Minimum 3 years professional experience within the healthcare, social services or community organization fields.
  • Minimum 2 years professional experience working with community programs and resources.
  • Minimum 1 year professional supervisory experience.
  • Relevant industry certifications.
