Cultural Community Health Worker (CCHW)

Position Summary:
We are looking for a passionate and dedicated Cultural Community Health Worker (CCHW) to support and uplift Black, Indigenous, and People of Color (BIPOC) communities. As a trusted connector, you'll provide culturally responsive outreach, health education, and care navigation, while addressing social determinants of health and advocating for individual and community needs.
You'll meet people where they are-in their homes, neighborhoods, hospitals, and community spaces-to help reduce health disparities, increase access to services, and promote human rights and social justice.
What You'll Do:
Provide health education and connect individuals to resources
Conduct home and community visits
Help people apply for health benefits and access needed care
Serve as a trusted advocate for underserved communities
Collaborate with families and partners to address complex social needs
Minimum Requirements:
High school diploma or GED
1+ year CHW or related community experience
Comfortable visiting homes, hospitals, and community settings
Strong empathy, communication, and collaboration skills
Proficient in Microsoft Office
Bilingual (English/Spanish, Arabic, Somali, or Pashto) preferred
