We are seeking a skilled Physical Therapist to join our team at Notre Dame Health Care Center's Long-Term Care and Skilled Nursing facility. As a Physical Therapist, you will be responsible for providing exceptional care to our patients and helping them achieve their rehabilitation goals.
Responsibilities:
- Develop and implement individualized treatment plans for patients
- Evaluate patients' progress and adjust treatment plans as needed
- Educate patients and their families on exercises and techniques to improve mobility and reduce pain
- Collaborate with other healthcare professionals to ensure comprehensive care for patients
- Maintain accurate and up-to-date patient records

Requirements
- Bachelor's or master's degree in Physical Therapy
- Current state license to practice Physical Therapy
- Experience working in a long-term care or skilled nursing facility preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team

$50 - $55/hour
