Job Description

Position Summary: Plans, organizes and coordinates the operations of the program by performing the following functions directly or through delegation to appropriate subordinate staff members. Additionally responsible for providing case management services to clients.

Principal Responsibilities:

  • Develops, implements and maintains departmental projects, policies and procedures, objectives, quality assurance and environmental standards, in conjunction with Program Director.
  • Reviews completed projects, makes recommendations, and reports on program activities to Program Director.
  • Assigns and coordinates work projects, as needed. Supervises assigned staff and social work interns, if applicable.
  • Assists with outreach and training components of the program, including presentations to community organizations and trainings to providers.
  • Interfaces with regional agencies to coordinate assessment and necessary paperwork, in accordance with Claims Conference policies and procedures.
  • Represents the organization at community meetings, committees, and inter-agency councils.
  • Performs client intake duties by phone or walk-in.  Takes information from client or collateral (e.g., family members, physician, etc.) to determine eligibility and need.
  • Becomes knowledgeable about home care rates and government subsidized home care programs in cities and states throughout the service area; works with home care vendors in the Northeast to secure home care rates, coordinate services and address service complaints as needed.
  • Works with all parties involved in the administration and utilization of home care, including but not limited to clients, family members and other collaterals, social workers and home care vendors to address concerns that arise around home care provision; advocates with local resources as needed to secure case management services for clients receiving home care through the program.
  • Maintains case notes and records, and required documentation, according to departmental standards and/or regulatory requirements.
  • Prepares statistical and other reports, as required.
  • Performs other related duties as assigned or requested.
  • Adheres to organization’s standards by completing annual mandatory trainings in a timely manner on topics including – but not limited to – Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Adheres to the organization’s policy in regards to absenteeism and appearance and health and safety standards.

 Salary Ranges: $60,000 - $62,400 per year commensurate with experience. 

Job Competencies & Minimum Qualifications:

  • Master's degree in Social Work (MSW) or related field, and two years of human services experience, preferably in the field of aging
  • NYS license and registration to practice as a Licensed or Certified Social Worker in New York State preferred
  • May supervise the social work and/or support staff in the program; interviewing and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems
  • Computer literate
  • Excellent verbal and written skills
  • Excellent customer service skills
  • Able to multi-task and work independently with a great attention to detail

Working Conditions/Physical Demand:

  • Business office environment with phone and computer use.
  • Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
  • Occasional exposure to outdoor weather conditions, and risk of exposure to pathogens and unsanitary conditions in client’s homes.
  • Must be able to attend meetings, other out-of-office functions and travel to client homes by using public or private transportation or other conveyance.  Valid driver’s license, car registration and automobile insurance, as applicable.
