Description

The Center for Elders’ Independence is a PACE (Program of All-Inclusive Care for 
the Elderly) organization that uses an interdisciplinary team approach to care planning 
and care implementation for the purpose of providing high quality, affordable, integrated 
health care services to the elderly, including an Adult Day Health Center, and promoting 
autonomy, quality of life and the ability of individuals to live in their communities.


POSITION: The Health Information Coordinator compiles, processes, and manages 
health information records by ensuring patient records remain accurate, complete, 
current, confidential, and are compliant with federal and state regulations, HIPAA, and 
CEI policies and procedures. The Health Information Coordinator works in conjunction 
with the interdisciplinary team to ensure complete integrity of patient information. 
Duties and Responsibilities

The rate range for the Health Information Coordinator at Center For Elders Independence is $ 22.28/hr to $ 33.43/hr. Wages/salary are based on the market for the Health Information Coordinator position, as well as experience, skills, abilities and work history


1. Enters data and makes changes in CEI’s electronic health record, including
documentation of POLST.
2. Scans and files documents to electronic record in a timely manner.
3. Conducts audits of the records to maintain accuracy and completeness of the 
electronic health record, consistent with federal and state regulations, HIPAA, 
and CEI policies and procedures.
4. Identifies and reports documentation deficiencies to team members and to 
Leadership to ensure deficiencies are resolved. 
5. Obtains outside medical records from external providers / other facilities for 
current participants including new enrollees’ medical records and enrollment 
documents and informs all necessary parties once received; scans documents 
into electronic record.
6. Monitors clinic fax inbox for any clinical reports, renames reports and ensures 
timely delivery to appropriate provider.
7. Responds to requests for access to participant medical records from participant 
or family, and other outside sources, including obtaining appropriate approvals
for release prior to releasing the records.
8. Maintains monthly deactivation lists/enrollment status in electronic record.
9. Collaborates to prepare records for CMS or other regulatory agency audit.
10.Medical Records Tracking to include filing, storage and retention of active and 
closed medical records and health information including recording submission of 
medical records to off-site storage.
11.Ensures adherence to HIPAA compliance standards at all times to ensure 
confidentiality of participant’s private information.
12.Performs other duties as assigned in a positive and helpful manner.


Minimum Qualifications
1. 2+ years’ experience in medical records position preferred.
2. Demonstrated ability working in an electronic medical records environment.
3. Prefer individuals with one of the following: Registered Health Information 
Technician (RHIT), Registered Health Information Administrator (RHIA), or 
Bachelor’s Degree in Health Information Management.
4. Knowledge of HIPAA compliance standards.
5. Excellent skills with Microsoft Office Suite.
6. Strong organizational skills and attention to detail.
7. Strong communication and interpersonal skills.
8. Demonstrated ability to handle confidential information
