Description:
Music Therapist- Hospice- Per Diem - serving Greene County and Southwestern Albany County areas
Community Hospice is looking for a qualified Music Practitioner/Therapist to join our team! This is a temporary grant funded position to provide complimentary therapy services in rural areas.

What you will do:
The Music Practitioner/Therapist is an important member of our health care team, serving Hospice patients in Home Care, Nursing Home and Inpatient settings.
Responsibilities:
- Responsible for direct care and evaluation of patients, with resultant formulation and implementation of treatment program as appropriate to the specific needs of that patient.
What you will need:
- A current license to practice as a Music Therapist Or Certified Music Practitioner in the state of New York required
- Prefer two years professional experience in music discipline; previous home care, hospice or health related experience important.
- Possession of a valid New York State driver's license and available automobile for travel throughout the service area.
- Experience working with computers, including laptops, email, cell phone and basic software programs along with confidence in providing electronic clinical notes and documentation regarding patient sessions.

Pay Range: $24.85 - $31.00 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
