The Social Worker will be a key participant in member interdisciplinary teams, weighing in on a host of medical, physical, social, and emotional assessments and Plans of Care. Additionally, this professional will oversee a number of core programming and therapeutic responsibilities.Pay: $21.00-$25.00
Bonus: $1000.00 to be paid in 3 increments:
- $200.00 - After completing 90-day probation
- $400.00 - After 6 months of employment
- $400.00 - After one year of employment
MINIMUM QUALIFICATIONS:
- A minimum of 18 years of age required.
- Bachelor’s degree in social work or related human services field. Master’s degree in social work or related field is preferred.
- One year experience working with adults in the mental health field
- Valid driver’s license required.
DUTIES AND RESPONSIBILITIES:
- Serve as the primary liaison to community hospitals, other social service agencies, mental health centers, intensive case-management units, etc., regarding any referral for admission and/or discharge planning and follow up contacts.
- Participate in activities addressing program review and development, quality assurance, utilization review; and staff development and training.
- Coordinate admissions and re-admissions into the facility and discharges to a less restrictive level of care.
- Provide counseling to individuals receiving services and their families regarding facility policies and procedures, social needs, and concerns about disability, death, dying or other emotional concerns.
- Provide clinical supervision/guidance to Recovery Support Practitioners and other staff involved in running group or advanced clinical treatment.
- Perform responsibilities associated with administrator on-call on an as needed basis, including:
- Securing replacement staff
- Responding to crisis situations
- Visiting the program during nonscheduled work time to resolve clinical, personnel, or programmatic issues.
- Establish a positive, supportive working relationship with family members involved with the recovery of the individual receiving services
- Coordinate the referral process for potential admissions, conduct pre-admission screening, and review all required paperwork for new admissions.
- Conduct initial and annual psychosocial assessments, including an initial family/psychosocial history.
- Perform annual assessments on all individuals receiving services.
- Provide financial management assistance as needed to individuals receiving services, including:
- Completion of Financial Management Agreement forms on an annual basis
- Operation of the “LTSR Bank”
- Conduct financial/budget training groups for individuals receiving services as appropriate
- Assist individuals receiving service in benefit acquisitions
- Utilize safe crisis management techniques to manage situations involving agitated and/or hostile behaviors from individuals receiving services, without the use of physical restraints.
- Serve as liaison to the Court regarding any scheduled hearings on involuntary commitment and complete paperwork required for Court within the prescribed time frames.
- Communicate any complaints and/or violations of the rights of individuals receiving services to senior management. Review investigation findings and develop a plan to prevent reoccurrence.

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
PHYSICAL DEMANDS AND WORK ENVIRONMENT:
- Regularly required to lift 10 lbs.
- Must be able to lift a minimum of 15 lbs.
- Must be able to pull a minimum of 20 lbs.
- Must be able to squat, kneel, crawl, crouch, climb, and stoop.
- Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.
- Required to regularly stand and walk.
- Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing, and lifting.
- Vision requirements include close vision, distance vision, and peripheral vision.
- Must be able to talk and hear.
