Located in beautiful Petaluma, Ridgeway Post Acute is a skilled nursing facility committed to quality care, teamwork, and resident dignity.

Key Responsibilities:

• Develop and implement individualized care and discharge plans in collaboration with the interdisciplinary team

• Serve as a liaison between residents, families, and community agencies

• Conduct psychosocial assessments and maintain thorough documentation

• Participate in care conferences and ensure residents' rights and needs are respected

• Provide support during admissions, discharges, and transitions of care

• Promote resident well-being through advocacy and person-centered support

Qualifications:

• Bachelor's degree in Social Work or related field (required)

• Minimum 1 year of experience in a skilled nursing or long-term care setting (preferred)

• Knowledge of discharge planning, Medicare/Medicaid regulations, and community resources

• Strong organizational, communication, and documentation skills

• Passion for working with elderly and post-acute populations
