Licensed Practical Nurse (LPN)
Location:Erie, PA | Schedule: Full-Time | Industry: Community Health

What You'll Do:
Conduct and document patient health histories
Assist providers in delivering high-quality, patient-centered care
Manage patient flow, including triage and follow-up care
Coordinate referrals to specialists and community services
Provide health education to patients and caregivers
Collect lab specimens (e.g., blood, urine, sputum)
Support infection control, safety standards, and supply use
Occasionally assist with front office duties (scheduling, discharge, etc.)
Float between Community Health Net locations as needed
What You Bring:
Valid Pennsylvania LPN license & current CPR certification
High school diploma or GED
Strong assessment, organizational, and communication skills
Knowledge of medical terminology and basic insurance types
Ability to use computers and electronic health record systems
