Position Function Summary
Under the supervision of the Patient Support Services, Manager/HCH Manager the Community Care Coordinator, assists the Lead in improving awareness of and access to La Familia Health (LFH) medical, dental, behavioral, support, and healthcare for the homeless for patients and target non-patients.
Essential Responsibilities
- Assists with community partner collaboration around patient access and social determinants of health by supporting partner communication, attending meetings, and connecting with new partners when possible;
- Explores community resources and maintains a working knowledge of agencies providing services to improve patient health equity.
- Helps to coordinate community health programming, including food security, nutrition, wellness, and other events.
- Provides health education and motivational interviewing to target patients and community members; identifies health barriers and needs, navigates to LFH services and "social determinants of health" resources.
- Provides Medicaid enrollment, patient registration, and scheduling services in target community health settings.
- Understands health disparities in Santa Fe County and their root causes.
- Supports health equity of patients and target community members in emerging public health issues, such as COVID-19 pandemic, by implementing target health education interventions.
- Disseminates health education information and materials across LFH sites and to community leaders.
- Assists in coordination and entry of qualitative data collection for patient satisfaction and ongoing community health needs assessment activities.
- Tracks, prepares, and submits all required patient and activity reports accurately and in a timely manner.
- Exhibits a wholesome, appropriate role model for LFH and patients.
Requirements
Education, Experience and Skills/Training
- Bachelor's degree, preferred, or four (4) years' health care related experience, required.
- Prefer candidate with experience in one of the following areas: medical or dental front office work, inventory management, customer service, event planning, medical assistants, community health workers, patient navigation or care coordination, etc.
- CPR Certification, required.
- Fluent in both English and Spanish, required.
