Position: Licensed Clinical Social Worker (Palliative Care & Hospice)
Status: Per Diem, Part Time OR Full Time
Hourly Rate: $45–$55/hour
Coverage Area: South Riverside to Temecula, including Hemet

Your Role

  • Emotional Support & Counseling: Assess psychosocial needs and provide compassionate counseling and crisis intervention.

  • Care Planning: Collaborate with nurses, physicians, and other team members to create individualized care plans.

  • Family Education & Advocacy: Help patients and families understand resources and navigate complex systems.

What We’re Looking For

  • Education: Master’s Degree in Social Work (MSW) required.

  • Experience: Prior experience in healthcare, hospice, or palliative care settings preferred.

  • Skills: Strong interpersonal, organizational, and communication skills. Able to work independently.

  • Requirements:

    • Valid LCSW license in California

    • NPI number

    • Reliable transportation, valid driver’s license, and auto insurance
