Description
Job Objective: This position is responsible for managing an assigned caseload of patients and their caretakers that are engaging in Parent Child Interaction Therapy (PCIT) at various Community Care of West Virginia locations. The PCIT case manager is responsible for performing initial PCIT intake assessments, assessing needs, developing, implementing and reviewing treatment plans, collecting data, providing supportive psychotherapy, coordinating patient appointments, taking and relaying clinical information to PCIT providers, coordinating PCIT trainings, coordinating PCIT room schedules, performing prior authorizations and coordinating services for individuals and families served. The PCIT case manager provides services and assists the mental health professionals in the development and delivery of services to individuals challenged by mental illness and/or addiction.
Position Responsibilities/Essential Duties:
Manages the PCIT wait list and is responsible for initial triage of referrals after assignment from referrals coordinator and subsequent intake of PCIT patients to assist in determining appropriateness for PCIT.
Assists in educating patients about the commitments required for PCIT after being accepted into the program.
Administers ECBIs as directed.
Maintains data graphs.
Accepts patients from the referral wait list and facilitates scheduling with the PCIT provider. Calls and gathers pertinent information about appointments, discusses what to expect on the day of appointment and engages in pre visit planning. Completes case management assessment on all patients entering caseload.
Obtains all necessary previous treatment records after obtaining consent.
Develops appropriate treatment/service plans with patients as the means for implementing appropriate services and developing effective alliances with patients.
Provides on-going supportive therapy and/or case management functions under the general supervision of the assigned provider in accordance with the problems, needs, and the strategies identified within the treatment plan in order to help the patient achieve the stated goals and objectives.
Provides face-to-face reviews with patients and/or relevant staff on a regular basis, the progress made in achieving treatment goals so that the treatment plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency of reviews will be determined by the treating provider and treatment plan.
Documents all contacts on a timely basis including phone calls, face-to-face interviews, meetings with treatment providers, maintains case records in accordance with regulatory standards and requirements.
Assists the patients in obtaining appointments with external specialists and setting up patient use of community services.
Refers patients to community resources as needed. Establish and maintain relationships with identified service providers.
Ensures that referrals are addressed in a timely manner.
Maintains ongoing tracking and appropriate documentation on referral tracking.
Communicates with behavioral health staff any issues or any need of further follow up and immediately reports urgent concerns to the appropriate personnel.
Contacts the patient and/or family member regarding any pending referrals, and or diagnostic testing.
Sends appropriate notifications, reminders and letters to patients communicating referral status. Review details and expectations about the referral with patients.
Ensure the patient's electronic health record is up to date with information on specialist consultations including documentation of scheduled appointment, results from specialist and treatment recommendations.
- Meets regularly with immediate supervisor and assigned providers as a means of enhancing professional growth, reviewing case management activities and addressing administrative issues. Maintains close communication with the psychiatrist and support staff regarding medication compliance, side effects of medication, and medication changes and alerts these members of the staff to any changes in patient which may suggest the need for intervention.
Participates in regular interdisciplinary staff meetings held at position location.

Coordinates and assists with PCIT training
Maintains inventory and orders PCIT supplies as needed.

Follow proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur.
Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service.
Consistently evaluate work and determine if further steps are needed to meet patient expectations.
Ensure compliance with regulatory standards.

Qualifications
Education/Qualifications/Experience:
Must possess a combination of behavioral health work experience or relevant education and training which indicates the possession of knowledge, skills and abilities to perform essential job duties.
Bachelor's degree in social work, psychology or human services related field is required.
License & Certifications- None required.
Experience
Two (2) years of experience in a healthcare setting, preferably behavioral health.
Experience working directly with adolescent populations and families.
Experience with Medicare and Medicaid population, insurance providers and telephonic engagement.
Demonstrated experience in working with an underserved population.
Experience with child protective services and court systems preferred.
Experience with, and working knowledge of, PCIT strongly preferred.
Knowledge/Skills/Abilities:
Knowledge:
Working knowledge of the nature of serious mental illness and addiction and related treatment modalities, intervention and techniques; of different types of assessments and their uses in treatment planning; consumers' rights; local community resources; of patient record documentation requirements; and of patient treatment plan development and implementation.
Abilities:
Demonstrated ability to interview and assess patients, using appropriate assessment tools, and observe, record and report on an individual's functioning; to read and understand assessments, evaluations, observation, and use in development treatment plans; to establishing effective working relationship with staff as well as with relevant community organizations; and interact positively with patients and their families. Demonstrated ability to work independently, as well as collaborate with multi-disciplinary teams and health care providers. Must be able to multitask and manage PCIT cases and providers at several locations.
Skills:
Must demonstrate excellent interpersonal communication skills, the ability to engage with varying patient populations. Must be flexible and adaptable to change. Strong analytical, organizational and time management skills, able to work with little supervision while handling numerous projects at once. Effectively use electronic health records (EHR) to document patient interactions, progress and other relevant information.
Physical Demands/Working Conditions:
Work is performed in a professional medical office setting, business casual dress environment.

Ability to see information in print and/or electronically.
Job requires standing, walking, hearing, reaching, talking, and occasional lifting up to 25 pounds.
Incumbent will be exposed to virus, disease, and infection from patients and specimens in working environment.
Incumbent will be required to work at any facility and be responsible for own transportation.
Incumbent may experience traumatic situations including psychiatric, dismembered and deceased patients.
