Position Summary:
As a Social Worker, you will play a vital role in supporting the emotional, social, and psychological well-being of our residents. You will collaborate with residents, families, and interdisciplinary teams to ensure personalized care plans meet the unique needs of each individual. This position is perfect for a compassionate professional who thrives on making meaningful contributions to the lives of others.
Key Responsibilities:
Conduct psychosocial assessments to identify residents' needs and develop individualized care plans.
Provide emotional support and counseling to residents and their families.
Serve as an advocate for residents, ensuring their rights, needs, and preferences are respected.
Coordinate discharge planning and transitions of care, collaborating with external providers as necessary.
Facilitate communication between residents, families, and staff to address concerns and resolve issues.
Assist residents and families in accessing community resources and support services.
Maintain accurate and timely documentation in compliance with state and federal regulations.
Participate in interdisciplinary team meetings to provide input on resident care.
Qualifications:
Bachelor's or Master's degree in Social Work (BSW or MSW) required.
Current state licensure as a Social Worker.
Experience in skilled nursing, long-term care, or a healthcare setting preferred.
Strong interpersonal, communication, and problem-solving skills.
Ability to handle sensitive situations with empathy and professionalism.
Knowledge of state and federal regulations related to social work in healthcare settings.
