Job Description
Summary:
Under the direction of the Program Manager, Service Coordinators will maintain rapport with clients, coordinate case management, complete data collection/analysis, and participate in continued program development and evaluation.
Schedule:
Tuesday-Saturday (time varies)
ESSENTIAL DUTIES & RESPONSIBILITIES
Including but not limited to the following list. The company reserves the right to add, delete, or modify responsibilities at any time.
- Caseload: Maintain a caseload of youth aged 10-17, referred through the Chicago Police Department, ensuring the highest level of client services is provided. Perform assessments with each client to determine their strengths and needs. Support client-led goal setting and achievement.
- Supervision: Participate in regular one-on-one supervision and team case-planning meetings. Participate in other group supervisions and training as necessary.
- External & Interagency Relationships: Maintain regular communication and rapport with coworkers, partner agencies, and referral sources in the community beneficial to the client and/or company.
- Recreational & Restorative Justice Activities: Assist in the development and execution of events in the community that are beneficial to clients, their families, and their communities.
- Court Attendance: Attend court with clients as necessary. Provide the Court with case plans or updates as necessary.
- Transportation: Transport youth as necessary.
- HIPPA: Full access to client information. Confidentiality must be maintained.
- Quality Improvement: Participate in ongoing continuous improvement efforts of the company.
- Diversity & Cultural Awareness: Ability to work effectively with diverse communities and stakeholders. Maintain cultural competencies and cultural humility.
Required qualifications:
- Bachelor's degree required
- 3 year's experience with case management skills
- Effective communication
Preferred Qualifications:
- Experience with youth from 10-17 years of age
- Community experience
- Willing to work with the Chicago Police Department
