Position Summary: An Maternal Fetal Medicine Nurse Navigator is a specialized registered nurse who guides expectant mothers through their pregnancy journey.

Essential Duties and Responsibilities:

Summary:

• Assisting with care coordination between inpatient OB/NICU units and outpatient MFM clinic.
• Participate in provider-led visits and carry out delegated tasks such as performing nonstress tests (NSTs).
• Collaborate with the interdisciplinary care team to define care objectives and assess patient and family healthcare, education, and psychosocial needs.
• Ensure continuity and connection with pediatric care for newborn post-delivery
• Develop and regularly update individualized care plans; monitor patient outcomes and promote self-management tools for sustainable well-being.
• Promotes and supports the goals and objectives of the organization

Requirements:

• Currently licensed as a Registered Nurse in the State of Illinois.
• CPR.
• BSN degree preferred.
• Demonstrates clinical knowledge and skills in area of specialty.
• Prior supervisory experience preferred.
• Experience with OB care including prenatal visits and fetal surveillance (NSTs) required.
• Certification in area of specialty preferred.
• Exhibits personal characteristics and enhance the effectiveness of the role, including effective interpersonal communication skills and a proactive approach to problem solving.

Work Shift Details:

Days, Mon-Fri 7:30 - 4 (hours may vary on some days, based on clinic hours)

The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.
