The Transplant Social Work position involves evaluating pre-transplant patients through completing a full psychosocial assessment to identify candidacy for transplant.

Job Description

• Providing support for post transplant patients and families.
• Social Work utilizes community resources in the surrounding counties and states for transportation to post transplant appointments.
• Social Work completes proper documentation both pre and post transplant for patients and their supports.  
• Social Work is involved in multiple meetings with the interdisciplinary team. Support activities of Transplant Programs by providing counseling services and appropriate liaison activities for Kidney, Pancreas and Liver Transplant programs. Also have some involvement with the Hepatology and Surgical department.
• The position requires providing services to the adult population in a manner that demonstrates an understanding of the functional/development age of the individual served and the impact of acute/chronic illness on patients/families.
• Requires knowledge of community and hospital based resources and foster interdisciplinary teamwork.

QULIFICATIONS:

• MUST have Masters in Social Work (MSW)
• Medical Social Work experience Preferred
• Completing documentation in a timely manner
• Working with interdisciplinary team
• Communicating assessments to team members
• Communication skills with supports/families of patients
• Creative problem solving
