Job Description

Position Purpose

The Population Health Pharmacy Technician will demonstrate consistent application of

knowledge and skills to meet HEDIS Medication Adherence requirements that will improve

and maintain CMS Star Ratings. Assists the pharmacist in the execution of appropriate safe,

efficient, effective and cost-effective pharmaceutical care in the scope of their assignment.

This position will perform the different aspects of their position under the supervision of a

licensed pharmacist in conjunction with the Population Health Supervisor.

Essential Duties and Responsibilities

  • This position is responsible for the Medication Adherence HEDIS measures
  • Collaboratively work with the Revere Health Pharmacist and Population Health
  • Proactively conducts outreach to patients for medication compliance, answer

questions and requests, address any issues/barriers, refills and update prescriptions

  • Must interact with providers and staff via phone calls and internal messages/task
  • Professionally represent Revere Health to outside vendors and payers, including

outside pharmacies and insurance companies

  • Maintains a safe and clean pharmacy by complying with procedures, rules, and

regulations

  • HIPPA and infection-control policies and protocols compliance required
  • Maintains records by recording and filing physicians' orders and prescriptions
  • Prepares reports by collecting, tracking and summarizing information
  • Contributes to team effort by accomplishing related tasks as needed

Qualifications:

  • Current technician license to practice as a pharmacy technician
  • Current knowledge of state and federal regulations and statutes regarding the

practice of pharmacy and the distribution of approved drugs, controlled substances,

and investigational agent

  • HEDIS Medication Adherence experience or knowledge
  • Prior experience in a healthcare or high-volume customer service position
  • Excellent computer skills and knowledge
