Job Overview:

Day-to-day, this employee manages the daily clinical, financial, and quality performance and outcome of our Case Management Team. This manager assumes accountability as appropriate to the service line. This leader contributes to and supports the hospital's mission, strategic goals, policies, and procedures and demonstrates knowledge of and integrates the use of evidence-based outcomes for case management practice. The case management team is responsible for assessing the needs of the patient and family to coordinate, monitor, evaluate, and advocate for multiple services to meet the specific patient's complex needs. The case management team collaborates with the multidisciplinary team to support care progression and plan for a safe, appropriate discharge. The expectation of this role is to support case management processes, drive change management for performance improvement, and support the case management teams throughout the hospital.

Required: Certifications/Licenses/Experience:

  • A minimum of five (5) years of acute hospital care
  • A minimum of three (3) years of case management experience
  • Prior leadership experience
  • Registered Nurse with BSN; or Social Worker with MSW or LSW
  • Applicable Nursing or Social Work license, with the ability to practice in Indiana and Kentucky

Preferred: Certifications/Licenses/Experience:

  • Case Management Certification
  • Recent clinical experience in specific areas required for specialty clinical groups or clusters
