Job Description

WHAT YOU WILL DO:

This is an exciting nurse leader position responsible for managing overall care for residents on the long-term care units within The Hoy Center.

Key responsibilities include:

  • Providing clinical support, guidance, and oversight for resident-centered nursing care, especially in the memory care unit.
  • Demonstrating and promoting a culture of respect, passion, and professionalism.
  • Using clinical expertise to delegate and assign responsibilities and ensure quality, consistent care.
  • Leading a strong, survey-ready team focused on delivering top-quality care and supporting one another.
  • Ensuring compliance and promoting it among all team members.
  • Serving as a member of the Quality Assurance and Performance Improvement Committee (QAPI) and participating in interdisciplinary team meetings.
  • Overseeing and executing care plans responsive to risks such as falls, skin conditions, and other clinical issues.
  • Leading the memory care unit to ensure person-centered care, positive team engagement with residents, and honoring resident preferences and rights.
  • Creating an environment aligned with intentional dementia care approaches (Montessori method, Pathways to Engagement, etc.).
  • Partnering with Therapeutic Recreation to best meet residents' needs.
  • Collaborating with leaders across the community to address and resolve concerns.
  • Representing WCCB with regulatory agencies, family members, and guests.
  • Acting as the nursing representative at long-term care interdisciplinary care plan meetings, updating participants on nursing focus areas, problem-solving, and updating care plans as needed.
  • Assisting MDS Coordinators with resident interviews or other tasks as needed.

WHAT YOU WILL NEED TO BE SUCCESSFUL:

  • Highly motivated, energetic, able to work independently and under WCCB leadership.
  • Multi-tasker with the ability to manage pressure and adjustments with tact and ease.
  • Excellent communication skills, able to clearly articulate instructions and manage conflict professionally.
  • Must have Current RN licensure or BSN.
  • Must have at least three years of clinical experience (preferably in long-term care).
  • Must have at least three years of supervisory experience.
  • Strong computer skills, experience with Electronic Health and Medical Systems, Microsoft and/or Google applications.
  • Physical ability to climb or descend stairs, stand or walk for extended periods.
