Overview:
Are you a dedicated Nurse Practitioner or Physician Assistant passionate about making a difference in patient care? Sentara Medical Group is actively seeking a compassionate healthcare professional to join our office-based Comprehensive Care team in Norfolk, VA. Join an integrated network of providers committed to creating a positive patient experience in an environment where everyone feels seen, heard, valued, and supported.
Position Highlights:
- Schedule: 7:30AM - 5:00PM 4 days per week patient-facing, with one half-day dedicated to administrative time (remainder of the day off).
- Provide comprehensive/primary care to chronically ill patients in an office setting
- Average of 14 patients per day
- Focus on reducing hospital readmission rates through proactive patient management and education
- Conduct thorough assessments, diagnose conditions, and prescribe appropriate treatments
- Robust APP onboarding program consistent with didactic curriculum, specialty shadowing, competency-based evaluations, and mentorship

Minimum Requirements:
- Active NP or PA
- At least 5 years' experience as APP
