Job Description

Eventide Fargo in Fargo, ND is currently seeking a Full Time Licensed Social Worker - LSW.

The Social Worker provides leadership, assistance, and direction in meeting the needs of the residents, families, and staff relating to the social services provided at Eventide.

Ideal Candidate:

  • Must hold a Bachelor’s degree in Social Work by an accredited college or university.
  • Previous healthcare social work experience in long term care preferred. 
  • Must hold a current Licensed Social Worker license in ND.
  • Must be in good standing in all states in which licensure is or was held.

Essential Job Duties:

  • Assists with the process of resident admissions, transfers, and discharges. Completes all necessary documentation and screenings according to the federal, state, and local rules and regulations.
  • Is knowledgeable about Eventide’s offerings and provides more detailed information on Eventide’s continuum of care to prospective clients and families.
  • Provides information, counseling, referrals, orientation, and education for staff, residents, families, and the community and is available as a resource to other providers and interested parties.
  • Acts as a facility resource to provide intervention with difficult resident and family issues, utilizing grievance and conflict resolution skills.
  • Coordinates and facilitates resident care conferences including initial, quarterly, change of condition, special needs, and discharge conferences.
  • Assures social services documentation is completed according to federal, state, local, and facility policies and procedures.
  • Assists in the planning of resident care services as they apply to the psychosocial needs of the residents and families.
  • In conjunction with resident care team, constantly assesses the appropriate placement of current residents on individual units.
  • Participates in meetings to further enhance the social services program.
  • Assumes other responsibilities as requested by supervisor.
  • Continually updates knowledge base and skills by attending in-services, continuing education programs, and interdepartmental staff meetings. Serves on committees as requested.
  • Updates department policies and procedures as needed.
