- Provide physical therapy services to patients, including evaluation, treatment planning, and implementation of treatment plans.
- Develop and implement individualized care plans based on patient needs and goals.
- Monitor and document patient progress, adjusting treatment plans as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Educate patients and their families on exercises and techniques for at-home care.
- Stay up-to-date with advancements in physical therapy techniques and research.

Requirements:
- Valid license to practice as a Physical Therapist (PT).
- Experience in Home Health is preferred.
- Knowledge of medical terminology, anatomy, and physiology.
- Strong interpersonal and communication skills to effectively interact with patients and healthcare team members.
- Ability to assess patient needs and develop appropriate treatment plans.
- Compassionate and patient-centered approach to care.
