Job Title: Behavioral Health Care Navigator

Compensation (Based on experience and qualifications): Minimum $19.25

Location: Roseland Clinic 120 W. 111th St. Chicago, IL 60628

Schedule: Monday- Friday 9:00am- 5:00pm with a Saturday Rotation 8:00am- 1:00pm

We are seeking a compassionate and detail-oriented Behavioral Health Care Navigator to join our healthcare team. As a Behavioral Health Care Navigator, you will play a critical role in identifying and tracking care provided to patients with substance use and mental health disorders. You will be responsible for internal and external outreach to educate patients on medication compliance, relapse prevention, and safe use of Medication Assisted Treatment (MAT) for avoiding opioid misuse.

Responsibilities:

  • Collects and evaluates all educational material related to substance use and mental health treatment, prevention and education
  • Act as a coordinator of care for all patients identified with SUD diagnosis or who are participating in MAT services in CFHC clinics.
  • Works collaboratively with Chicago Family Health Center professional staff, serving as an active part of the patient care team and collaborating with interdisciplinary staff including licensed behavioral health providers, medical providers, dental providers, and support staff.
  • Will make and/or facilitate referrals to partner agencies providing more intensive SUD care - including intensive outpatient, group therapy, inpatient medical detox, and inpatient rehabilitation.
  • Will conduct outbound phone campaigns for patients hospitalized for SUD or MH reasons, or identified by MCOs as having a BH need and registered as a patient of CFHC with a need to be brought in for appointment.
  • Actively track and monitor a registry of patients in SMI (Severe Mental Illness) care, SUD care and/or MAT care.

Qualifications:

  • A minimum of a High School Diploma or equivalent.
  • CADC (Certified Drug and Alcohol Counselor) certification required.
  • CPR Certification will be required and maintained.
  • Experience working with patients with substance use disorders and mental health conditions required.
  • Experience working with suboxone (buprenorphine) or other forms of Medication Assisted Treatment (MAT) required.
  • Knowledge of activities and regulations related to substance use care in SUPR (Substance Use Prevention and Recovery)-licensed facilities required.
  • Knowledge of activities and regulations related to mental health care in specialty mental health facilities required.

Work Arrangement: This role is eligible for a "hybrid remote" work arrangement. If hybrid, it is expected this person would be able to meet for "in person" meetings when needed or at the request of others. In-person meetings may be required on a regular basis (weekly, monthly, etc.).

Requirements

Education and Experience
