Key Responsibilities
- Screen new clients in the primary care setting using advanced digital tools.
- Implement immediate interventions and innovative referral services as needed.
- Administer cutting-edge psychosocial assessments and crisis interventions.
- Deliver brief, impactful interventions using motivational interviewing and digital strategies.
- Conduct comprehensive clinical evaluations and counseling sessions.
- Maintain accurate and detailed client records and reports.
- Coordinate with community agencies for seamless service integration.
- Develop and implement client-centered treatment plans.
- Utilize data-driven approaches for continuous client assessment and treatment plan adjustments.
- Participate in monthly strategic meetings with clinical supervisors.
- Document all client interactions within a digital framework.
- Stay informed on behavioral health innovations and research.
- Engage in voluntary family planning services in compliance with FQHC requirements.
Minimum Qualifications
- Master's degree in social or behavioral sciences from an accredited institution.
- Current Texas license (LCSW, LPC, or LMFT) with active Medicare, Medicaid, and other reimbursement programs participation.
- Minimum of 2 years of experience in assessment, brief interventions, and psychotherapy.
- Skilled in managing multiple priorities and making tech-savvy decisions.
- Proficient in leveraging digital tools for client engagement and service delivery.
- Ability to thrive in a fast-paced, innovative healthcare environment.
- Bilingual in Spanish to accommodate patient population.
