The Masonic Homes of California will soon unveil its new skilled nursing and memory care services on our beautiful campus in a new, state-of-the-art building designed with the latest innovations in skilled nursing and memory care. We are a Continuing Care Retirement Community that provides homes to Masons, their wives and widows. You will be able to develop Social Services systems and programming for our existing Assisted Living and Independent Living Residents and build your program and help obtain our license for our new Skilled Nursing that includes Memory Care.
We can accommodate up to 70 Independent and Assisted Living Residents and will be able to accommodate 32 Skilled Nursing and Memory Care Residents.
POSITION SUMMARY:
The Social Service Designee plans, develops, organizes, evaluates and directs the overall operation of the Social Services department maintaining compliance with federal, state, and local guidelines and regulations, Masonic Homes policies and procedures, and standards of care for specialty practice. The Social Service Designee is a member of the interdisciplinary and management team of the nursing center. The Social Service Designee is responsible for fostering a climate, policies, and routines that enable residents to maximize their individuality, independence, and dignity. This climate shall provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.
DUTIES AND RESPONSIBILITIES
- ADMINISTRATIVE:
- Review facility policies and procedures as part of the facility’s interdisciplinary team (IDT) to assure compliance with state and federal regulations.
- Participate in reviewing and setting policies concerning resident care and quality of life.
- Participate in developing facility psychosocial policies.
- Develop, maintain and utilize a listing of current community resources that are useful to residents and their families/significant others.
- Participate in quality assurance interdisciplinary team meetings.
- Understand and meet all government requirements for social service documentation.
- Document progress in meeting the psychosocial needs of residents.
- RESIDENT ADVOCACY:
- Work with residents, families, significant others, the interdisciplinary team and administration to promote and protect resident rights and the psychological well-being and of each resident.
- Identify community changes and opportunities such as legislation, regulations and programs that affect nursing home residents.
- Coordinate ancillary services including but not limited to vison, dental, audiology and podiatry services.
- Schedule appointments with health care providers an arrange transportation as needed.
- Oversee grievance process and coordinate with administrator and the IDT for resolution.
- Oversee the facility theft and loss program.
- PSYCHOSOCIAL INTERVENTIONS:
- Complete a psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
- Provide timely and accurate completion of Social Services portion of the MDS, Care Area Assessments (CAAs), resident care plan, and progress notes as well as other forms as required in order to comply with federal and state regulations and facility policies and procedures.
- Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social service interventions.
- Provides guidance with end of life decision making, facilitating communication between patients, families, providers, the IDT and Physicians, supporting a compassionate approach to the process of transitioning to palliative care and hospice.
- Work closely with the facility mental health provider to ensure that all resident psychosocial needs are identified, referrals are made, and services are provided.
- Ensure or provide therapeutic interventions, support and education to assist residents/family members/significant others in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs.
- Maintain contact with the resident, legal representative, and other family members approved by the resident or legal representative, involving them in the resident’s total care and providing them with information on the resident’s status as requested or appropriate.
- Coordinate resident room changes.
- Lead facility behavior management committee. Coordinate care with social worker, psychologist and/or psychiatrist as needed.
Requirements
- Requires strong assessment; planning; and goal development skills.
- Requires the ability to communicate and resolve conflict with physicians, other health care team members, community agencies, clients and families with diverse opinions, values and religious/cultural ideas.
- Requires knowledge of community resources and entitlement programs.
- Requires knowledge of state and county adult protective legal mandates.
- Three years' experience in skilled nursing setting
- Social Service Designee Certificate .
- Active CPR certification
