Job Description

Job Title: Care Management Specialist- RN or LPN

Reports to: Director of Population Health

Department: Population Health

FLSA Class: Exempt

Summary: The Care Management Specialist- RN provides operational support to the Care Management and clinical teams to provide support and high-quality care for Hunter Health Clinic (HHC) patients. In collaboration with the Director of Population Health and Care Management team, the Care Management Specialist- RN is also responsible for maintaining compliance with all federal grants, HRSA, HIPAA, and HHC policies and procedures.

Essential Functions:

  • Assists in developing and implementing the Care Management program by reviewing utilization, discharge planning, transition of care, communication with hospitals and resources, chronic care case management, and care plans related to patients' social drivers of health.
  • Improves health outcomes by implementing changes to the care management processes and assisting with the development and updating of policies and procedures.
  • Coordinates patient discharge and post-discharge actions per program requirements.
  • Collects the patient's physical and mental wellness needs, preferences, and abilities to facilitate improved clinical interactions.
  • Coordinates care plan implementation and revision for high-risk patients across providers to support patient health outcomes and accountability.
  • Collaborates with care teams to ensure value-based care gap closure and improve value-based care data collection to satisfy program requirements.
  • Partners with providers and care teams to schedule and complete designated visits.
  • Collaborates cross-departmentally to ensure compliance and assists patients with enrollment and scheduling in a variety of programs including chronic care management, remote monitoring of hypertension control program, transportation services, Prescription Assistance Program (PAP), etc., based on program requirements.
  • Documents attribution-based patient data into electronic health record (EHR) and value-based care platforms, as directed.
  • Performs duties consistent with clinical skills training, as needed.
  • Prepares reports by collecting, analyzing, and summarizing results, data, and trends.
  • Participates in monthly meetings with accountable care organizations and similar companies to improve care management program outcomes.
  • Updates job knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.
  • Travels when necessary to meet operational needs.
  • Performs all other duties as assigned.

Qualifications:

  • Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the state of Kansas is required.
  • 1-3 years of experience in a medical clinical or hospital setting is preferred.
  • Prior experience with Electronic Health Records software and Patient Registration is preferred.
  • Care Management experience is preferred.
  • Bilingual abilities are preferred, but not required.
