Key Responsibilities

• Facilitate child and family team meetings and coordinate care with guardians and professionals.

• Assist clients in identifying and accessing resources for independent, healthy living.

• Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.

• Provide direct services including case management and daily living support.

• Conduct assessments to inform treatment planning and monitor progress.

• Serve as a liaison between clients and service providers, ensuring continuity of care.

• Advocate for clients within community systems and manage crisis situations as needed.

• Participate in organizational initiatives such as quality assurance and safety activities.

Experience and Education Qualifications:

• Bachelor's degree in a Human Services field from an accredited institution.

• Minimum of two (2) years of relevant experience.

• Strong communication and interpersonal skills.

• Proficiency in computer systems and Google applications.

• Ability to manage multiple priorities and maintain confidentiality.

• Knowledge of trauma-informed care and sensitivity to diverse populations.

• A positive, adaptable attitude and commitment to teamwork.
