Key Responsibilities
Facilitate child and family team meetings and coordinate care with guardians and professionals.
Assist clients in identifying and accessing resources for independent, healthy living.
Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.
Provide direct services including case management and daily living support.
Conduct assessments to inform treatment planning and monitor progress.
Serve as a liaison between clients and service providers, ensuring continuity of care.
Advocate for clients within community systems and manage crisis situations as needed.
Participate in organizational initiatives such as quality assurance and safety activities.
Experience and Education Qualifications:
Bachelor's degree in a Human Services field from an accredited institution.
Minimum of two (2) years of relevant experience.
Strong communication and interpersonal skills.
Proficiency in computer systems and Google applications.
Ability to manage multiple priorities and maintain confidentiality.
Knowledge of trauma-informed care and sensitivity to diverse populations.
A positive, adaptable attitude and commitment to teamwork.
