SCOPE OF ROLE
Reporting to the Supervisor of Behavioral Health Services, the Qualified Mental Health Specialist, Community-Based Family Therapy (QMHS-CBFT) delivers mental health services to children, adolescents, and adults in home, school, and community settings. The QMHS-CBFT supports clients with emotional or behavioral challenges by promoting stability, independence, and improved functioning. The QMHS-CBFT connects individuals and families to community resources, including transportation as needed, and advocates for client needs. Using a client-centered, strengths-based approach, this position engages clients in treatment, coordinates care across systems and supports those at risk for out-of-home placement or psychiatric hospitalization. The QMHS-CBFT builds strong, collaborative relationships with clients, families, and Signature Health team members.
HOW YOU'LL SUCCEED _____
- Diligently develop, prepare, and present treatment program materials to clients.
- Plan, coordinate, and provide reliable, clinically appropriate transportation to clients for medical/behavioral health appointments, community resources, and other essential services, ensuring safety, professionalism, and punctuality.
- Maintain clear communication and professional boundaries with all clients.
- Act as a subject matter expert in developing support networks and serve as a family advocate by coordinating with social service agencies.
- Identify client and family strengths and needs to support the development and implementation of Care Plans, Comprehensive Assessments, Treatment Summaries, Safety Plans, and Risk Assessments.
- Encourage, monitor, and follow up on client engagement and participation in all treatment plan activities.
- Collaborate effectively with the client's treatment team and participate in regular meetings with internal and external stakeholders to coordinate care.
- Create an outstanding experience for adult and child clients by connecting them to needed educational, medical, vocational, housing, and daily living resources.
- Provide crisis intervention and stabilization services in the community as needed.
- Serve as a liaison with inpatient units to ensure continuity of care.
- Maintain timely and accurate electronic documentation in accordance with Medicaid, OhioMHAS, CARF, and agency standards.
- Participate in community outreach, quality assurance, utilization review, and all assigned staff meetings and development programs.
- Complete all required compliance training and meet agency productivity and documentation expectations.
- Adhere to agency policies and procedures, including HIPAA and confidentiality standards.
- Demonstrate consistent attendance, punctuality, and efficiency in alignment with Signature Health policies.
- Other duties as assigned.
Requirements
KNOWLEDGE & EXPERIENCE
- High School Diploma or equivalent required. Bachelor's Degree in related social services or criminal justice field preferred.
- Valid unencumbered Ohio driver's license and proof of driver's insurance required.
- American Heart Association (AHA) Basic Life Support (BLS) certification required.
- 3+ years of previous case management experience providing service to criminal justice populations or relevant experience preferred.
- CDCA preferred.
- Strong computer skills with Microsoft, Excel, etc.
- Knowledge of the community, community organizations, and community resources.
- Knowledge of ASAM levels of care, ability to assist client in obtaining residential, detox, and supported housing services in the community.
WORKING CONDITIONS
- Work is normally performed in a typical interior/office/clinical work environment.
- While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
- Possible exposure to blood borne pathogens while performing job duties.
- Requires periods of sitting, standing, telephone, and computer work.
- Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
- Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
- Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
- Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
- Possible exposure to blood borne pathogens while performing job duties.
- Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
- Sufficient dexterity to operate a PC and other office equipment.
